Scheduling Your Practical Test

Please review all the information on this page with your instructor before completing the form below. Please use a computer rather than a mobile device since this is a long and complex form and requires you to upload a PDF of your test results

This information on this form is required because the FAA mandates that all practical tests be authorized through the Designee Management System (DMS) before they can be conducted.

Tests can be scheduled up to one month in advance. PLEASE NOTE: Your date is not confirmed until you receive a confirmation email from me. 

If your preferred date is unavailable, I will contact you by email to offer the next available appointment.

Important: Please upload a copy of your written test results with your submission.

NOTE: Sport Pilot applicants may use a valid U.S. driver's license in lieu of a medical certificate (per §61.23(c)(2)). If your aircraft has simplified flight controls (e.g., Ercoupe without rudder pedals), please note this in the comments field when scheduling. Your certificate may carry a limitation per 14 CFR §61.45(g)(4)(ii).
